Potty Training?

How do you know when your child is ready to be potty trained? I think that most kids begin potty training in the 2-3 range, but how can you tell when the time is right for your kid? Then, when you do decide that it's time, how do you go about actually potty training them? Do you use a rewards system? The family that I babysit for reinforces the use of the potty with 3 M&M's, which seems to be working great. What other methods could be used to do this?
